Congratulations once again on earning a Tom Lantos Innovation in Community Service Award. 

One of the objectives of the awards program is to extend the reach of funded programs within the PH community, and your report--which is a requirement of the grant--will help us do that.

This report covers your project's accomplishments as well as its challenges and benefits. It will help others in the PH community build on your successes and lessons learned. 

Your report should be based on the project description and project goals/objectives described in your original proposal. And while we hope that everything went as planned, we are as interested in learning what didn't work as well as what did.
