Grant Reporting/Unused Funds

Successful grants offer models to other members of our community. In order to help others in the PHA community replicate the innovative programs funded through the Lantos Awards, those receiving grants will be required to provide a written report of what has been accomplished. The form may be filled out online or mailed in (for the hard copy please call +1-301-565-3004 x 760). The report will be due within 60 days of the completion of the grant or one year from the award, whichever is earlier.

  • Completed projects are to include the following funding attribution, "This project was supported by a Tom Lantos Innovation in Community Service Award. The Pulmonary Hypertension Association's Tom Lantos Innovation in Community Service Award is funded by Gilead." 
  • Grantees will work with PHA staff to seek design approval prior to printing if projects refer to PHA programs and services.

At that time, grantees will also be required to submit receipts or other records of spending for their project as able to . Any unused funds will need to be returned to PHA.

Grantees may also be invited to present their results of their work at a "best practices" workshop at PHA's International PH Conference or at PHA regional meetings.
