As an older teen with pulmonary hypertension (PH), you will want to prepare to transition from your pediatrician to an adult PH-treating doctor. This can be an exciting yet intimidating step, especially if you’re close with your current PH care team. You may also have to start taking on more of your medical responsibilities, from managing your medications to ordering prescription refills.
