Aurora Health Care is the largest private, not-for-profit integrated health care provider in Wisconsin. The PH program includes a multidisciplinary team with PAH RN coordinators, physicians, advanced practice providers, and a pulmonary hypertension service coordinator. It also includes a full complement of medical specialists, as well as support from psychiatry, social work, financial counseling, dietary, spiritual services, pharmacy services, palliative care, and a research department to help with clinical trials.

Dianne Zwicke, MD is a board certified cardiologist and the medical director of the PH program since 1985. She has over 30 years of experience of caring for patients with PAH. Sara Paulus is the main PAH coordinator and sees patients in clinic with the physicians. There are also 4 other advanced practiced providers that work in the heart failure program and cross cover and care for the PAH inpatients.
