Submit this form to request PHA advertising, food, activity and venue sponsorship, and Zoom access for your meetings.
Once your request is received, PHA will send postcards and emails to people in your area whose contact information is in the PHA database, and add your meeting to the online support group meeting calendar.

After every meeting complete the Meeting Report Form.
This form allows you to:
- share attendance numbers
- update member contact info
- submit PHA food sponsorship receipts and paperwork
- report any issues from the meeting
PHA Food Sponsorship Policy
PHA offers a food sponsorship program that provides funds to support group leaders for food and beverages at in-person support group meetings. You may also use funds for organized activities that bring together support group attendees. All PHA funded programs must be pre-approved by PHA and meet the required per person dollar amount.
Food and activity sponsorship program guidelines:
- Funds can be used for food and non-alcoholic beverages OR organized activities at meetings (e.g. bowling, movies).
- Provides up to $25 per person plus additional taxes, fees and tips not to exceed 20%, per meeting for a maximum of one meeting per month.
- Once per year, leaders may request up to $35 per person for a special meeting (e.g. a holiday party or summer picnic).
- The fund may not be used for alcohol.
- Requests for funds must be pre-approved by PHA.
PHA is grateful for the generosity of our community members whose donations allow us to fund support group activities. We ask leaders to join us in being responsible stewards of donor support and keep expenses under the $25 maximum when possible.
However, we recognize that food costs have increased rapidly and we will consider limited exceptions on request. If you are struggling to find food for your meetings, please contact PHA at 240-485-0777.
Meeting Venue Policy
PHA encourages support group leaders to find a free venue to hold meetings. If you are unable to find a free venue with the requirements needed to run a successful meeting, PHA will consider providing the following assistance.
- PHA will pay venue fees to host a support group up to $100 per meeting. Requests to cover higher venue fees will be considered case-by-case on request.
- Requests for funds must be pre-approved by PHA.
- If the venue requires proof of PHA’s nonprofit status, reach out to PHA for assistance.
How to Apply for Funds
Complete the Meeting Request Form. Your request for funds will be reviewed and you will receive an email regarding the status of the funds.
Third-Party Sponsors
PHA provides support group leaders with a list of pharmaceutical and specialty pharmacy company contacts. Many of these companies have medical professional speakers’ bureaus and can connect leaders with PH experts to present a PHA-approved slide deck. Companies who sponsor a meeting speaker may also provide food for the meeting without the leader needing to apply to the PHA fund.
Leaders are encouraged to use a variety of sources to find meeting speakers and activities. When working with a pharmacy or specialty pharmacy representative, ask if they will provide food for your meeting.
