To apply for PHA-accreditation, centers must complete an application that evaluates five key areas:
- Center director.
- Center coordinator.
- Program staff and Support Services.
- Facility.
- Clinical Research (if applicable).

Steps to apply
- Submit your application online: Interested programs can access the application portal at the bottom of this page. The application is detailed to help site reviewers understand the context, capabilities and scope of your PH program. The application takes about 45 minutes to complete and must be done in one sitting. It is highly recommended that all required documents are collected before you begin. If at any point you have questions about the application process, email PHCC accreditation.
- PHA staff review: After submission, PHA staff will review your materials to ensure all components have been completed, including letters of support. Within a week of your submission, they will contact you to schedule a brief call to review the process and next steps.
- Pre-review committee evaluation: Your completed application will then receive a formal review by the PHCC Pre-Review Committee, a panel of PH clinicians and allied health professionals who are criteria content experts. The committee ensures that accreditation criteria are adequately met, and that your program is prepared for an accreditation site visit.
- Site review: If the Pre-review Committee approves your application, PHA staff will contact you to schedule a site visit at a time that works for integral members of your program. A team of two reviewers, a PH physician and an allied health professional with extensive experience in PH diagnosis and management, will assess your site in person. Site reviews occur over one business day, typically from 8 a.m. to 3:30 p.m. local time.
- Finalize contracts: Please note that your site review will not be officially scheduled until you have signed a Business Associate Agreement and Statement of Work with PHA. Please email PHCC accreditation for questions regarding these agreements.
- Accreditation decision: After your site visit, the reviewers present their findings to the PHCC Review Committee. The committee will discuss, vote and notify your program of the final decision.
